Acknowledgement of Risks Form Instructions for Graduate/Professional Students or Groups traveling abroad that will include Graduate/Professional Students
Duke Graduate/Professional Students who wish to travel to a location on the Duke Restricted Regions List (RRL) must sign an "Acknowledgement of Risks and Potential Harms, Agreement to Mitigate Risks, and Waiver and Release of Liability" ("Acknowledgement of Risks Form") specific to their destination(s) prior to departure.
The Acknowledgement of Risks Form can be obtained by contacting the Global Travel Office at globaltravel@duke.edu at least 30 days in advance of the intended travel dates. Included in the request must be the travel dates and destination(s) - city and country - of the planned travel.
Once the request is received by the Global Travel Office, Adobe Sign software will be used to distribute and complete the Acknowledgement of Risks Form electronically. Because the form is specific to each student and is prepared only after submission of travel details to the Global Travel Office, the form is not available online to download independently.
If a Duke Graduate/Professional Student fails to comply with the Global Travel Policy, the Duke Provost may compel the Student to return home at the expense of the Duke Graduate/Professional Student, may deny the application of academic credit or recognition of work completed based on the travel, and will explore the initiation of Student Conduct proceedings for a policy violation.
Programs, units, and schools who provide support – whether it is financial, granting approval or granting credit (even if the granting of credit is post-travel) – of travel to a RRL destination must ensure that Graduate/Professional Students receiving their support have completed the Acknowledgement of Risks Form prior to providing any Duke-support for the travel.
All travelers are required to register travel in the Duke University Travel Registry in order to be considered in full compliance with Duke's Global Travel Policy, in addition to completing the Acknowledgement of Risks Form.
If a group of Graduate/Professional Students is traveling to the same RRL destination, the Global Travel Office can coordinate with a representative of the group a method of batch travel registration and form distribution.
